FOR PwD APPLICANTS

How do I know if I am eligible for other Transportation Services and

how do I apply?

 

Since the PwD Program is only available to you if your trip is not covered by other transportation services or programs, the following information is provided to you to assist you with alternate applications, if necessary.

 

Medical Assistance Transportation Program (MATP): 

Transportation for medical assistance recipients is available to a variety of medical destinations such as hospitals, doctor’s offices, rehabilitation facilities, pharmacies, etc. There is no cost to the consumer for this service. TransNet operates the MATP in Montgomery County and applications are available here or by calling 215-542-RIDE (7433).

 

Montgomery County Aging & Adult Services:

Transportation funded by the Montgomery County OAAS available for Montgomery County residents who are 60-64 years of age. The service is limited to 8 one-way local trips per month and there is a 25% co-pay required. If you are attending a County sponsored senior center, trips are not limited and a co-pay is not required. TransNet operates this service and applications are available here or by calling 215-542-RIDE (7433).

 

Human Services Development Fund (HSDF):

Transportation to medical and social services is available for persons who are 18- 59 years old. Service is limited to 2 one-way trips per month and co-pay is not required. This is an income eligible program and referrals must originate from the Montgomery County OAAS. TransNet operates this service and applications are available here or by calling 215-542-RIDE (7433).

 

Montgomery County Developmental Disabilities Program:

Transportation to and from adult vocational and day programs is available for persons who 21 years of age or older with mental retardation. Referrals must originate from the Montgomery County Developmental Disabilities Program. TransNet operates this service and applications must be made through the Montgomery County Developmental Disabilities Program by calling 610-278-3642.

 

SEPTA CCT Connect:

ADA complementary paratransit service is available to anyone who is functionally unable to use fixed-route buses, for all travel that begins and ends in SEPTA’s service area. This service is operated by SEPTA through private subcontractors and applications are available by calling 215-580-7145. Physical and/or cognitive assessments may be required. The base fare is $4.00.

 

Pottstown Area Rapid Transit (PART):

Complementary paratransit service is available to any Montgomery County resident who is functionally unable to use fixed-route buses and lives or is traveling in PUT’s service area. This service is operated by PART and applications are available by calling 610-326-5413. The base fare is $3.40.
